ACU Innovation & Consulting developed the 2023-2024 community health needs assessment and community service plan of Wyckoff Heights Medical Center in Brooklyn, NY. The needs assessment was comprised of 202 surveys with patients/community residents, 22 interviews with staff members, 10 interviews with leaders from community-based organizations and a comprehensive analysis of NYS and NYC data.
